DeSoto County’s Community Health Assessment (CHA) 2016 is a compilation of the most recent data that describes the state of our county’s health. The CHA is a collaboration of the Florida Department of Health in DeSoto County and the Health Planning Council of Southwest Florida. The CHA provides an in-depth look at health factors (behaviors, clinical care, and the physical environment) and the outcomes (length and quality of life) that are a result of our county’s status.
